Matt,
It is almost two years since you wrote this article. With regard to the first two points the suggestion to include address and phone number on every page - do you still consider that as important now as back then? We chose to 'hide' that information on our Contact page for a specific reason - every hit on that page is most likely someone planning to contact us and consequently a good indicator of 'conversions'. We have done really well for the keywords 'window replacement san francisco' so far. Your thoughts?

We are forgetting to mention about the "Google Local Business Referrals agents". These people literately go visit business by business documenting addresses, photos, phones, name contacts, websites, etc. I could bet Google takes this listings as priority. Maybe the reason why some small local business get on top Google listings (even if they don't have a websites, or reviews).

That's a good one, Dave. Anytime you can get IBLs with relevant anchor text -- in this case, with location signals -- you're doing yourself a favor. My list in this post is stuff you can do yourself on your own pages/site, so I didn't include things like anchor text from other sites -- but a good thing to keep in mind, regardless.

Get anchor text back links that include the location information along with a business/product/service description.

If you can add it in gracefully try adding a local keyword in the header tags as well. "Widget Makers of Saint Louis, MO" as opposed to just "Widget Makers"
